Fallen Gabriel: Grants key pieces with cost of 1 heart. If Fallen Gabriel holds both key pieces rerolls every item in the current room into another item from the Angel Room item pool which costs hearts. In Sheol or Cathedral teleports into Chest. 6 charges after rerolling items.

Isaac and Tainted Isaac: Uses The D6. 20% chance to use Devil D6 or Angel D6 instead. 6 charges after use.

Magdalene and Tainted Magdalene: Spawns a Heart. 25% chance to use Full Health. 4 charges after use.

Cain: Spawns a random pill or a Locked Chest. 4 charges after use.

Judas and Tainted Judas: Uses The Book of Belial. 60% chance to use the book twice. 30% chance to spawn a Black Heart. 4 charges after use.

??? and Tainted ???: Spawns 8 poops of any type. 4 charges after use.

Eve: Spawns 14 Dead Birds for the current room. 4 charges after use.

Samson and Tainted Samson: Activates Berserk!. 6 charges after use.

Azazel and Enty: Grants Brimstone for the current room. 3/4 charges after use.

Lazarus and Tainted Lazarus: Grants an extra life. 12 charges after use.

Eden: Activates a random active item, and grants a random passive item effect. Random charges after use.

The Lost: Grants an additional Holy Mantle for the current room. 4 charges after use.

Lazarus II: Reverses Lazarus into his original form. 6 charges after use.

Black Judas: Reverses Black Judas into his original form. One-time use.

Lilith and Tainted Lilith: Grants 2-4 familiars for the current room, and uses Box of Friends. 4 charges after use.

Keeper: Spawns 1-25 coins. 6 charges after use.

Apollyon and Tainted Apollyon: Voids all items in the room, and an additional passive item. 6 charges after use.

The Forgotten and Tainted Forgotten: As The Forgotten grants a temporary range boost; as The Soul grants a temporary fire rate boost. 4 charges after use.

Bethany: Spawns 10 random wisps. 10 charges after use.

Jacob and Esau: Grants 2 random passive item effects to both characters for the current room. 4 charges after use.

Tainted Cain: Spawns 3 random pickups. 6 charges after use.

Tainted Eve: Spawns 2 Blood Clots. 4 charges after use.

Tainted Azazel: Grants flight, and full Brimstone for the current room. 4 charges after use.

Tainted Eden: Grants 5 random passive items, and rerolls all of Tainted Eden's items. Uses The D6 and D20. One-time use.

Tainted Lost: Grants an additional Holy Mantle for the current room or an extra life. 4/20 charges after use.

Tainted Keeper: Converts the room into gold. 12 charges after use.

Tainted Bethany: Spawns 4 item wisps from the Secret, Devil, and Angel rooms. Wisps can be of active items. 20 charges after use.

Tainted Jacob: Chains every enemy in the room, including Dark Esau. When in ghost form, double charges, and grants Holy Mantle for the current room. 6 charges after use.

Gabriel: Grants Holy crucifix. One-time use.

Crice: Uses Console 10 times. Rarely can teleport into the I AM ERROR room. 3% chance to teleport to the final boss (Ultra Greed, or Home). 10 charges after use.

Bombaman: Grants one bomb item, and uses Mama Mega!. One-time use.

Stefan}: Spawns 3-5 cards. Only one can be picked up.

Sim: Spawns 5 Axes.

Frosty: Grants frozen tears, similar to Uranus, and uses XIII - Death card.

Tainted Frosty: Grants frozen tears, similar to Uranus, and warms up Tainted Frosty. If Tainted Frosty is too hot, then he will additionally get frozen.

Tainted Frosty (Ghost): Grants 1-7 special wisps, and has a small chance to instantly revert into his original form.

The Sheriff and Tainted Sheriff: Spawns an Ammo pack. 20% chance to use Soul of the Sheriff.

This item may appear within a Tainted Treasure Room as a counterpart of The Bible.
This item contributes towards the Bookworm transformation. Collecting 3 items from this set will make Isaac fire an extra tear 25% of the time.
Unlock: Defeat Mom's Heart as Fallen Gabriel.
